Niklas Dahl – Ärftlig epilepsi och kognitiva funktionsnedsättningar: Identifiering av sjukdomsmekanismer och nya kandidatläkemedel
Den långsiktiga målsättningen med vår forskning är att utveckla effektivare behandlingar för behandlingsresistent epilepsi samt för vissa former av kognitiv funktionsnedsättning som orsakas av definierade genetiska förändringar.
Olika former av epilepsi och kognitiva funktionsnedsättningar med tidig debut är båda globala hälsoproblem. Drabbade individer har en kraftigt sänkt livskvalité och effektiva behandlingsformer saknas. Diagnosgrupperna är heterogena men en majoritet av fallen orsakas av specifika genetiska avvikelser som ger bestående symtom.
I våra projekt använder vi modeller av specifika och tidigt debuterande typer av dessa sjukdomar som tagits fram från så kallade inducerade pluripotenta stamceller (iPSC).
Specifika målsättningar:
- Etablera och utveckla nervcells-modeller från iPSC för utvalda former av terapiresistent epilepsi och kognitiva störningar.
- Analysera dessa nervcellsmodeller för identifiering av sjukdomsspecifika mekanismer och biomarkörer på cell- och molekylnivå.
- Följa och normalisera uttrycket av sjukdomsspecifika biomarkörer i nervcellsmodellerna med kemiska bibliotek (s.k. ”drug-screen”). Driva de första stegen i utveckling och/eller optimering av kandidatläkemedel vid respektive tillstånd.
Bättre kunskap om sjukdomsmekanismer behövs
Utvecklingen av riktade och effektiva behandlingar vid medfödd epilepsi respektive kognitiva störningar är långsam i jämförelse med utvecklingen för flera andra sjukdomsgrupper. En begränsande faktor är bristande kunskap om sjukdomsmekanismer dels på grund av svårigheter att få tillgång till biologiskt material, dels för att det saknas relevanta modellsystem som återskapar sjukdomsprocesserna. Begränsningarna kan idag delvis överbryggas med en kombination av nya teknologier i form av inducerade pluripotenta stamceller (iPSC), modifiering av DNA med CRISPR/Cas9 (gen-sax) samt storskalig analys för bildanalys, DNA/RNA sekvensering m.m.
Modeller av nervsystemets sjukdomar för att klargöra sjukdomsmekanismer
Vi använder iPSC som får mogna ut till nervceller för att efterlikna sjukdomsprocessen både tvådimensionellt och tredimensionellt. Vi återskapar då de specifika celltyper eller strukturer i hjärnan som är drabbade vid respektive sjukdom. De tredimensionella modellerna är i form av så kallade hjärn-organoider som visas i figur 1.
Stamcellerna är etablerade från patienter med specifika former av epilepsi respektive utvecklingsstörningar som bekräftade genetiska orsaker. Vi har även inducerat sjukdomsorsakande genvarianter i iPSC från friska individer med CRISPR/Cas-teknik.
Nervcellsmodellerna analyseras med hjälp av bl.a. RNA-sekvensering, immunfärgning, bildanalys, elektrofysiologi m.m. Med modellerna har vi identifierat specifika cellulära avvikelser kopplade till bl.a. epilepsi vid Dravet sjukdom och Neurochondrin-brist. Vi har även klarlagt mekanismer av betydelse för nedsatt kognitiv förmåga vid Down syndrom och Mowat-Wilson syndrom.
”Drug-screening” av cellulära sjukdomsmodeller för läkemedelsutveckling
Sjukdomsassocierade cellulära och molekylära avvikelser (biomarkörer) används i nästa steg för att screena av sjukdomsmodellerna med kemiska bibliotek (s.k. “drug-screening”). För ändamålet har vi etablerat samarbete med Chemical and Biological Consortium Sweden (CBCS) och utvecklat ett experimentellt flöde med specifika infärgningar av nervcellsmodellerna (så kallad ”cell-painting”se figur 2). Det följs av bildbaserad cellanalys och artificiell intelligens (AI).
Målet är att identifiera en kemisk grundsubstans som kan korrigera sjukdomsassocierade förändringar i nervcellsmodellerna vid screening och som ett första steg vid utveckling av nya läkemedel.
