Individualized virtual type 2 diabetes care

We designed a study to evaluate the effect of an individualized written theory-based individual care plan for persons with type 2 diabetes and self-management deficits on glycemic control, health-related quality of life and the experiences of living with diabetes and support from the diabetes care. The aim is also to evaluate the patients’ experiences of the written care plan.

Project description

Patients with type 2 diabetes and HbA1c higher than 52 mmol/mol will be recruited from primary care unites in Region Uppsala. The intervention is based on the theoretical framework of Orem. The underlying assumption is that the nurse can facilitate the action of self-care by clarifying the person’s self-care requisites and to support the person to express this both verbally and in writing.

Recruiting diabetes nurses will attend a two-hour educational opportunity regarding the theoretical framework of the intervention. They will be encouraged to talk and reflect on the use of the written care plan within the group and with the study nurse in charge to firmly establish the process.
